-- Your greatest accomplishments over the past five years.
-- Name and address of the venue(s) where you showcase your work.
-- Frequency of your performances or exhibits.
-- Specifics on your next show.
-- A recent high-resolution digital photo of your work (with names of anyone in the photo).
-- Your Web address and phone number.
