John H. "Jack" Smith, 76, of Blue Grass, died Sunday, April 8, 2012, at his home.

Memorial services are 5:30 p.m. Thursday at McGinnis-Chambers, Bettendorf, with visitation 4:30 p.m. to services. Private burial is at Blue Grass Cemetery.

He was born Feb. 5, 1936, in Cordova, to John and Mary Durbin Smith. He married Mary Joe Waltz Aug. 2, 1957.

Surviving are his wife; children, Jacqueline, Dennis, Scott, David, Blue Grass, Jeffrey, Davenport; 14 grandchildren; one great-grandson; brother, Robert, Mt. Carroll, Ill.
